Caracalla visited the temple of Pergamene Asclepius as he moved east to confront the Parthians in 214-215 AD. There, he participated in its rites, which included sleeping within the temple precinct and having the dreams incurred thereby interpreted. To commemorate this event, a number of huge medallic bronzes, including this coin, were issued.

The attribution of a specific Julio-Claudian prince to each of these portraits remains a matter of conjecture. Mionnet believed them to be portraits of Gaius and Lucius Caesars, an interpretation the authors of RPC believed quite plausible. They do note, however, that the index to the von Aulock collection interprets this issue both as Gaius and Lucius, and Drusus and Germanicus, all of whom became potential heirs to the imperial throne. Barring any more conclusive evidence, the matter will remain unresolved.

Lysimachos made himself master of Ephesos in 295 and shortly thereafter changed the city's name to Arsinoeia in honor of his wife, the daughter of Ptolemy I. After the death of Lysimachos in 280, Arsinoe married her half brother Ptolemy Ceraunus and finally her full brother Ptolemy II. She became the first Ptolemaic ruler to enter the Egyptian temples as "temple-sharing goddess" and was revered by the later Ptolemies.

This type is unpublished, but similar pieces are known for Abydos and Kebren (and also possibly Gergis; see Triton XVI, lot 437), where the obverse features the civic badge of the city (eagle at Abydos and ram at Kebren) and the reverse has a facing gorgoneion in incuse square. The style of the gorgoneion here is also consistent with these other issues.
